We want to make sure that our dogs are sent to people who truly will care for
them. If you are interested in purchasing a Corgi, please take the time to fill out the following information so that it may be
evaluated. Once approved, we will fill out the applicable parts of the
sales contract and send it to you. All information will be kept private.
(Note: because we do not yet
have an error page for incompleted questionnaires please read and review your form carefully before submitting, and fill it out
to your best ability. If you do make a mistake and already have submitted the form, just fill out your name and the fields you wish to fix and resend. Thank you!)

At least a city is required for the address. E-mail must be valid. Because it's a good way of
organizing information that we can look back at easily for reference without tying up the phone-line, we prefer contact via e-mail.
Lastly, in the event of possible loss of an e-mail (hasn't happened yet) it might be a good idea to make a copy of any
long commentaries or remarks you might have that you don't want to have to re-write, using something simple such as Notepad or Wordpad
or in an e-mail you can send to yourself.
